Background
Doctors Without Borders India (DWBI)/Médecins Sans Frontières provides medical assistance during natural and manmade disasters through establishment of healthcare centres, implementing medical programs for critical illnesses on its own or with partners, working for creating medical awareness and capacity building and carry out/sponsor research to provide strategic input on relevant medical treatment options in India.
DWBI offers assistance to people in need, irrespective of race, religion, gender or political affiliation.

Location: Zokhawthar, Champhai (Mizoram)

Main purpose of Post:
Provide outpatient and/or inpatient medical care to patients/beneficiaries according to adapted and updated medical knowledge, DWBI protocols, values and universal hygiene standards to improve the patients / beneficiaries health conditions.
Functional and Hierarchical Lines:
The position is hierarchically and functionally answerable to Project Medical Referrent (PMR) respectively.
Responsibilities/Accountabilities:
•Apply medical knowledge and skills to diagnose and prevention. Carry out outpatient and inpatient consultations, prescribing the necessary treatment respecting DWBI protocols.
•Keep the patient and/or his/her family informed about the illness and provide appropriate explanations about the treatment to follow, checking they have understood.
•Follow up the evolution of the hospitalized/IDP patients, through daily visits, consultations and examinations, prescribing the necessary treatment following DWBI protocols, deciding whether they can be discharged or transferred to other departments – in collaboration with other doctors- and informing their family about the patient’s evolution.
•Check and control the rational distribution of medicines and equipment under his/her responsibility and take care the quality, disinfection and sterilization of the medical material. Assure general compliance with standing hygiene standards.
•Participate in the collection and analysis of epidemiological data, checking its validity and informing the line manager or project coordinator about any problem or complication of the patients’ illness, medical error and monitors the proper functioning of the department, equipment or material.
•Ensure ongoing training of the medical/paramedical multidisciplinary team in order to optimize the quality of care.
•Knows and ensures all DWBI medical protocols are followed and implemented, checking universal precautions are followed at all times and reducing bio-hazard risks and improving infection control. Ensures professional confidentiality is respected.
•Manage the team under his/her direct responsibility according to DWBI HR policies and procedures, supervising their performance, organizing and scheduling shifts and rotations, directly participating in emergencies and on calls if necessary.
Registration in Indian Medical Council, and Mizoram State Medical Council is essential.
Some other Accountabilities:
•Manage DWBI clinic by applying medical knowledge and skills to diagnose, and screen for mental health issues and carry outpatient consultations along with managing patients in a holistic approach by keeping the patient and/or his/her family informed about the illness and providing appropriate explanations about the treatment to follow, and ensuring that they are involved in the management.
•Be flexible to attend emergencies 24X7 at the DWBI clinic.
•Ensure patients are prescribed medications strictly as per DWBI and National Medical guidelines and maintaining the Rational prescription analysis record every months.
•Screen, Identify and link the patients to the appropriate centre for suspected TB/HIV and follow up with their health status.
•Have and apply medical knowledge and skills for screening,diagnosing and management for NCD cases and maintaing close followup of the same. Actively participate in the implementation of NCD program in the project.
•Make sure that the DWBI clinic maintains good quality and confidential medical records of patients treated by it and health information and statistics of the respective patients.
•Ensure that all records are maintained accurately including NCD + HIV/TB register and inform PMR in case of any discrepancy.
•Provide quality, ethical and person centered clinical care to survivors of sexual and gender-based violence presenting at the DWBI clinic such as appropriate treatment following protocols, accurate documentation of the results of medical examination and injuries sustained, while keeping confidentiality.
•Complete the weekly OPD, nutritional and SGBV tally sheets and submit them to the Data team every Monday morning.
•Have updated and clear understanding of NCD, MHGAP, SGBV, Malnutrtion and Vaccination Protocols.
•Maintain updation of OPD, logistic and NCD database and logbook, with Data team support.
•Support data team whenever required for HMIS medical data for quality records.
•Report to the PMR weekly on patient diagnosis, numbers and challenges, seen in the consultations.
•Support PMR in updation and reporting of weekly Punto and monthly Sitrep or any medical report.
•Refer patients to secondary healthcare providers based on the referral framework created specifically for the project after obtaining approval from PMR/ Referral MD focal point.
•Ensure that the referral logbook is updated with accurate diagnoses and follow-up details are updated daily.
•Bimonthly physical follow up with patients who are being referred to the tertiary hospitals(Aizwal) to ensure they receive the services needed.
•Visit the secondary health facility once a month to review the patient’s treatment, and investigations, discuss with the treating doctor about the management plan support the referral nurse whenever required and update PMR accordingly.
•Work closely with other members of the medical team staff (Nurses for triage and patient flow, counsellors, Health promotion, Lab technicians and Medical data processing officer) to ensure quality medical care.
•Ensure that you are updated about all the Lab tests which are available in the DWBI clinic must know the procedure and in case the need arises should know how to perform it.
•Participate and update in clinical as well as medical team meetings.
•Ensure IPC protocols are in place, maintain the IPC while performing any procedures and address the safety needs of the patients.
•Maintain surveillance in the DWBI clinic and notify on a regular basis if there is any alert which needs to be investigated.
•Participate in assessment and emergency response when necessary and according to the workload and priority.
•Actively be involved in paricipation during any Mass casuality Incident response and or any outbreak response.
•To be a part of the emergency response team in case of any outbreak or emergencies.
•In case of any outbreak conduct a rapid health assessment wherever required, provide a detailed report with feedback, and discuss with PMR if there is a need for any intervention
•Evaluate each intervention provide feedback and discuss with the PMR for more efficient intervention.
•To make relevant data and make medical reports of each assessment/intervention and provide them to PMR.
•Inform PMR in case of any change of pattern in the consumption of medications.
•Plan and organize regular training for DWBI medical staff at least once a week.
•Plan and organize training for all the DWBI staff at least once in 2 months on relevant disease outbreak awareness or Basic life support.
•Ensure confidentiality while handling documents related to patients and ensure psychological support throughout the consultation.
•Support clinical/community activities with any additional task assigned, such as conducting training and sensitization sessions.
•To support PMR for medical review, management of the staff under staff health protocol, with additionally supporting in maintaining line list and consolidation of the staff health documentation and confidentiality of the same
•Maintain a flexible approach and respond to the needs of the project even outside of the job description.
•Maintain surveillance in the DWBI clinic and notify on a regular basis if there is any alert which needs to be investigated.
•Support clinical/community activities with any additional task assigned, such as conducting training and sensitization sessions.
•Whenever required fill in the gap in case of absence of other medical staff
•To respect DWBI internal regulations.
Requirements:
Education
•Medical Doctor diploma.
Experience
•2-year experience minimum as a Medical Doctor or in clinical work (can be within medical training). Desirable in tropical medicine, or post-registration experience in Public Health, obs and gynae, pediatrics, A, infectious diseases, HIV/AIDS/STDs, TB, general practice, general medicine or minor surgery
Languages
•English- essential. Local language: Mizo, Chin, Tedim, Zomi, Falam desirable.
